Individuals may ask for help from CHN by calling the agency to discuss their needs and by completing a CHN program application for breast cancer patients or a CHN program application for non-breast related cancer patients. The application must be signed by the individual (or legal designee) and a health care provider who can attest to his/her cancer diagnosis.

CHN understands that not every individual may have the same need. Examples of the type of assistance that will be considered include:
• Travel expense (gas cards)
• Co-pays for prescriptions or health care providers
